In the Shop Area near the MCV-1300 CNC vertical machine, where employee operated a 1-ton Ingersoll Rand overhead hoist that the safety latch on the hook had been tied back to hold the latch open.      b.  In the Assembly Area where employee operated a 1/4-ton CM overhead hoist that was missing the safety latch on the hook.     c.  In the Assembly Area where employee operated a 500-lb. CM overhead hoist that was missing the safety latch on the hook.
