29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) On or about July 17, 2019, at the southeast side of the commercial building, the second level of the supported scaffold used by employees to scratch stucco surface on front parapet wall was not fully planked, exposing employees to a 12 foot fall hazard.   b) On or about July 17 2019, at the northeast side of the commercial building, the second level of the supported scaffold used by employees to apply plaster stucco on the wall was not fully planked, exposing the employees to a 10 foot fall hazard.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  a) On or about July 17, 2019, at the southeast side of the commercial building, the tubular framed scaffolding system 2 tiers high being used by employees plastering stucco was missing mudsills in all frames, exposing employees to fall, collapse and struck by hazards.  b) On or about July 17, 2019, at the northeast side of the commercial building, the tubular framed scaffolding system 2 tiers high being used by employees plastering stucco was missing mudsills in all frames, exposing employees to fall, collapse and struck by hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a) On or about July 17, 2019, at the southeast northeast sides of the commercial building, employees used the tubular components of the scaffold to access the two levels of the frame scaffold.  b) On or about July 17, 2019, at the northeast sides of the commercial building, employees used the tubular components of the scaffold to access the two levels of the frame scaffold.
