OSH Act of 1970, Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee(s) were exposed to drowning and crushing hazards:     7605 St. Charles Rock Road, Bel-Nor: Employer is exposing employees to drowning and crushing hazards.  The employees are permitted to operate commercial lawn equipment near a creek and at an incline without being provided required formal training.         On 07/23/2019 an employee who had not been trained operated a zero-turn commercial lawn mower (Lastec Articulator, Model 3373-Serial No. 31040408) within 10 feet of a creek in the Eleven Rough section of the golf course.  It was the first time the employee operated the specific lawn mower.  The employer did not have a copy of the manufacturer's operation manual available for the employees.  The employee did not receive formal training prior to operating the specific lawn mower.  The employee was discovered in a creek still attached to the seat belt of the equipment.  The employee drowned.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:     7605 St. Charles Rock Road, Belnor: Employer is exposing employees to drowning, electrical, crushing, struck by, amputation, and chemical hazards.  The employees work at the establishment where hazard assessment of the work place is not conducted.       On 07/23/2019 employees were exposed to hazardous conditions such as, but not limited to, working with exposed defective electrical equipment and operating commercial lawn equipment close to a creek, on soggy grounds or inclined environments without having written and certified hazard assessments.   An employee who did not have access to a written and certified hazard assessment drowned when he operated commercial lawn equipment close to a creek on an inclined soggy ground.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:     7605 St. Charles Rock Road, Belnor, Maintenance Shop: Employer is exposing employees to hazardous conditions involving pinch points.  The employees are permitted to operate a bench grinder in a defective condition exposing the employees to pinch point hazards.     A week prior to 07/23/2019 employees used a bench grinder (STS Model-IDS 450) which was available for use at the time of inspection.  The work rest of the bench grinder extended 5/8 inch from the stone wheel exposing employees to in-going pinch point.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:      7605 St. Charles Rock Road, Belnor, Maintenance Shop: Employer is exposing employees to conditions involving struck by hazards.  The employees are permitted to operate a bench grinder with no upper guard.     A week prior to OSHA inspection employees used a bench grinder (STS Model-IDS 450) without an upper guard (tongue guard).  The bench grinder with stone wheel was available for use in the same condition observed on 07/23/2019.

Employees are permitted to work where electrical hazards exist.     On 09/12/20019 an employee was exposed to electrical hazards where there were exposed energized electrical parts.  A junction box and duplex receptacle outlet with exposed parts such as, but not limited to, energized strands were not prevented from accidental contact by the use of covers.

The employees are permitted to work where there are electrical wiring hazards.     On 09/12/2019 employees were exposed to electrical hazards where they used electrical boxes which had missing knockouts to power cleaning equipment for different types of golf carts in the Cart Barn.

The employees are permitted to work where there are electrical wiring hazards.           On 09/12/2019 an employee was exposed to electrical hazards where a 5-receptacle relocatable power tap was attached to a flexible cord set (extension cord). The extension cord was used as a substitute for fixed wiring to supply power to equipment such as, but not limited to, computers and monitors.
