The employees are permitted to operate commercial lawn equipment near a creek and at an incline without being provided required formal training.         On 07/23/2019 an employee who had not been trained operated a zero-turn commercial lawn mower (Lastec Articulator, Model 3373-Serial No. 31040408) within 10 feet of a creek in the Eleven Rough section of the golf course.  It was the first time the employee operated the specific lawn mower.  The employer did not have a copy of the manufacturer's operation manual available for the employees.  The employee did not receive formal training prior to operating the specific lawn mower.  The employee was discovered in a creek still attached to the seat belt of the equipment.  The employee drowned.
