The scaffold was not erected under the direction of a competent person qualified in scaffold erection; 7/18/19. b) Workplace, 11 Vail Ave., East Quogue, NY- Two employees were performing roofing work while standing on a scaffold with a single 10 foot long and 9 1/2" wide plank. The scaffold was not erected under the direction of a competent person qualified in scaffold erection; 7/18/19. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
