29 CFR 1910.132(d)(1)(iii): The employer did not select PPE that properly fit each affected employee: Employee(s) were exposed to a fall hazard. The employee working on the mezzanine level unloading product was unable to correctly wear the only harness provided for fall protection. The harness provided was too small for the employee and could not be worn as designed to be effective. The fall height from the open mezzanine gate to the bottom floor was approximately 13 feet 3 inches.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Employees were exposed to a fire hazard. A power strip located in the server room on the mezzanine level had 3 additional power strips plugged into it and were all in use. Overloading power strips in this manner could result in a fire which would be a hazard to all employees working on-site.
