29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  At the jobsite located at 8870 North Shannon Avenue, building 5, Kansas City, Missouri 64153, employees engaged in roofing activities were exposed to health hazards from dermal, ingestion, and inhalation exposure to hazardous chemicals. The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work. Hazardous chemicals used at the site included but were not limited to gasoline.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At the jobsite located at 8870 North Shannon Avenue, building 5, Kansas City, Missouri 64153, employees were exposed to serious struck-by eye and face injuries. The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. The employee was working with assorted hand tools while conducting roofing work with the potential for eye injury without the use of eye protection.

29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  At the jobsite located at 8870 North Shannon Avenue, building 5, Kansas City, Missouri 64153, employees were exposed to serious struck-by hazards. Employees engaged in roofing work more than 20 feet in elevation was throwing roofing debris off of the roof and onto the ground below.

At the jobsite located at 8870 North Shannon Avenue, building 5, Kansas City, Missouri 64153, employees were exposed to serious fall hazards. Employees were working at elevation without the proper use of guardrail systems, safety net systems, or personal fall arrest systems. Employees were exposed to fall hazards of over 25 feet to the ground below.

Employees were exposed to a fall hazard of 20 feet to the ground below.
