29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Jobsite, 302 Stump Drive, Belle Vernon, PA 15012: Observed on or about July 24, 2019, employees working on the ground and first level of a two story single family house were not wearing protective helmets. The employees were working in areas where there was a possibility of head injury from falling objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Second floor, 302 Stump Drive, Belle Vernon, PA 15012: Observed on or about July 24, 2019, employees working on, at, above, or near second floor wall openings, where the outside bottom edge of the wall openings was approximately 23 inches above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. The employees were exposed to a fall hazard of approximately 20 feet to the ground below. b) Front porch, 302 Stump Drive, Belle Vernon, PA 15012: Observed on or about July 24, 2019, employees where using a Werner brand portable extension ladder to access the front porch and first floor of the house. Guardrails were not installed on open sides of the porch. Employees were expose to a fall hazard of approximately 10 feet to the ground below. c) Roof, 302 Stump Drive, Belle Vernon, PA 15012: Observed on or about July 24, 2019, employees engaged in framing activities on top of a two story single family house where not protected from falling by the use of a guardrail system, safety net system, or a personal fall arrest system. The employees were exposed to a fall hazard of approximately 25 feet to the ground below.
