29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Ascent Roofing Solutions, LLC located at 4979 Vicksburg Lane, Hilliard, OH, 43026: On and preceding July 25, 2019, the controlling employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower level were protected from falls. Roofers were conducting removing shingles on a roof that varied from 10-18 feet with the employees working at different levels. The employees were not utilizing proper head or fall protection which exposed the employees to falls up to 18 feet.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Ascend Roofing Solutions, LLC located at 4979 Vicksburg Lane, Hilliard, OH, 43026: On and preceding July 25, 2019, the controlling employer did not ensure that employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects utilized head protection. Roofers were conducting removing shingles on a roof that varied from 10-18 feet with the employees working at different levels. The employees were not utilizing proper head protection which exposed the them to struck by hazards to the head.
