29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points:    Ollie's Distribution Center, Commerce, GA - On or about July 18, 2019, in the warehouse near the outbound dock door exit area, employees were exposed to a fire hazard when the means of egress pathways and aisles were partially blocked by stored material that reduced the exit path below 28 inches.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.37(b)(1): Each exit route was not equipped with operable lighting that to allow employees, with normal vision, to see along the exit route.    Ollie's Distribution Center, Commerce, GA- In the warehouse area near the outbound dock doors, employees were exposed to a fire hazard when emergency exit route lighting did not work. The employer failed to maintain and test the emergency lighting fixtures on a periodic basis to ensure the lights would function properly during a facility power failure, on or about July 18, 2019.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.176(a): The employer did not assure that aisles and passageways were kept clear where mechanical handling equipment was used and that permanent aisles and passageways were appropriately marked:    Ollie's Distribution Center, Commerce, GA - On or about July 18, 2019, in the warehouse area, employees were exposed to a struck-by hazard while walking and working near unmarked aisle ways where forklifts traveled with loads that partially blocked the operators vision.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(n)(4): The employer did not assure when the loads being carried obstructs forward view, powered industrial truck drivers travel with the load trailing:    Ollie's Distribution Center, Commerce, GA - On or about July 18, 2019, in the warehouse area, employees were exposed to a struck-by hazard when forklift drivers were allowed to carry loads that blocked their view in the forward direction while operating in warehouse aisle ways that were not marked for employee protection.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
