29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person. On or about 8 July 2019, and at times prior thereto, a job-made wooden trash box was utilized for movement of materials and construction debris with a forklift, exposing the operator of the forklift to the hazard of being struck by pieces of the trash box and the materials or debris from elevated heights.
