29 CFR 1910.1026(d)(2)(i): The employer using the scheduled monitoring option did not perform initial monitoring to determine the 8-hour time-weighted average exposure to chromium (VI) for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area: (a) For the employees welding stainless steel in the welding department on or about August 7, 2019 and times prior thereto, the employer did not conduct initial monitoring to determine employee exposure to Chromium VI for employee welding in steel containing chemicals such as, but not limited to, 308 Stainless Steel welding rod containing 19.5-22% Chromium and 301 Stainless Steel sheet metal containing 17.577% Chromium.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) For the employees working in the metal fabrication area on or about July 25, 2019 and times prior thereto, the employer was using a power strip with a UL rating for residential use that was plugged into an extension cord that was plugged into the power cord of a shop fan to power the fan.
