29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   (a) On July 25, 2019, CFL Starters & Alternators Inc. did not ensure that strain relief was provided to the flexible cords located in the sandblasting room. The flexible cord connected to the sandblasting machine was pulled exposing the wires. The flexible cord for the welding machine plugged into the electrical outlet was pulled apart and covered with electrical tape.    No abatement certification or documentation is required for this item.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) would be met:  a) On July 25, 2019, CFL Starters & Alternators Inc. did not develop or implement a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) that would describe or include at least the following:  1) Requirement for labeling and other forms of warning; 2) Safety data sheet availability; 3) Employee information and training; 4) A list of hazardous chemicals known to be present in the workplace;  5) Methods to inform employees of the hazards on non-routine tasks; and  6) Methods to provide other employer(s) access to safety data sheet; information on any precautionary measures and the labeling system used in the workplace.  Employee(s) were exposed to hazardous chemicals including but not limited to stoddard solvent, naphthalene, toluene.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and (2) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area:  a) On July 25,2019, CFL Starters & Alternators Inc. did not provide effective information and training on the health hazards to employee working with Crystal Clean 106 Mineral Spirits, which contained stoddard solvent, naphthalene and toluene.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical.  a) On July 25, 2019, CFL Starters and Alternators Inc. did not maintain copies of the Safety Data Sheets (SDS) for Crystal Clean 106 Mineral Spirits, which contained stoddard solvent and was used to wash alternators parts in the sandblasting room.   CFL Starters and Alternators Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.1200(g)(8), which was contained in OSHA inspection number 1374605, citation number 1, item number 3, issued on April 16, 2019 and became final order on or about May 02,2019  with respect to a workplace located at 4929 W. Grand Avenue, Chicago, IL 60639.   No abatement certification or documentation is required for this item.
