Pine Island Rd. Plantation, FL, an employee working in a trench at approximately 4-feet in depth without a safe means of egress, was exposed to a cave in.

Plantation, FL, an employee working in a trench at approximately 4-feet in depth without the spoil being at least 2 feet from the edge of the trench, exposing the employee to a struck by hazards from excavated materials falling or rolling into the trench or excavation.
