29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a. Lenin Milla Construction site located at 1000 Solomon Square Building #4 in Delaware, Ohio: On or about July 25, 2019 the employer did not ensure two employees working from an aerial lift while installing windows, were protected from falling approximately 12 feet 7 inches to the ground.
