29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields: 51 Maple Street, Agawam, MA: On or about July 26, 2019, employees working in an excavation protected by shields were not protected from cave in hazards when entering or exiting the protected areas. The provided egress ladder was located outside of the protective shield in an unprotected area of the excavation.
