Employees working on the scaffold were exposed to fall hazards of 15 feet 2 inches due to numerous deficiencies in the scaffolding such as but not limited to missing uplift pins, lack of guardrails around scaffold work platforms, work platforms were more than 14 inches from the face of work, work platforms were not fully planked, base plates were not secured, scaffold platforms were not 18 inches wide, and there was no ladder access.
