On or about July 25, 2019 and at times prior thereto, employees were replacing a metal roof on a multi-story building, at heights ranging from 6 feet to 30 feet above a lower level without the combination of their personal fall arrest and warning line systems, or the use of a safety monitor, exposing employees to the hazards of falling from heights 6 feet or more from a lower level.
