29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Saul Roofing LLC, Buford, GA - On or about 07/24/2019 at 4329 Old Hamilton Mill Rd, Buford, GA, employees were observed exposed to fall hazards while performing roofing operations at a height over 25 feet from the ground level with a roof pitch of 4/12. Employees were not wearing fall protection systems.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  Saul Roofing LLC, Buford, GA - On or about 07/24/2019 at 4329 Old Hamilton Mill Rd, Buford, GA, employees were observed exposed to fall hazards while performing roofing operations at a height over 25 feet from the ground level with a roof pitch of 4/12 when the employer failed to install anchors onto the roof and the lanyards were wrapped around the beam and attached to itself.

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  Saul Roofing LLC, Buford, GA - On or about 07/24/2019 at 4329 Old Hamilton Mill Rd, Buford, GA, employees were observed exposed to fall hazards while performing roofing operations at a height over 25 feet from the ground level with a roof pitch of 4/12. Some employees were wearing only rock-climbing harnesses.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Saul Roofing LLC, Buford, GA - On or about 07/24/2019 at 4329 Old Hamilton Mill Rd, Buford, GA, employees were observed exposed to fall hazards while performing roofing operations at a height over 25 feet from the ground level with a roof pitch of 4/12. No training was provided to employees on the proper use of fall protection. Employees did not know at what height is required the use of fall protection.
