29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about July 25, 2019, the employer did not develop, document and utilize machine specific procedures for machines with more than one energy source. When performing servicing of CNC Machine #15 at the Northfield holdings, LLC dba Northfield Industries, LLC location the employee was exposed to the unexpected energization of equipment due to a lack of procedures and failure to control all potential sources of energy. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) On or about July 25, 2019, the employer and the host employer Northfield Holdings, LLC dba Northfield Industries, LLC did not inform one another of their lockout tagout procedures.
