Employees engaged in material handing and production activities using the Toyota forklift, Model 5FGC25, serial number 5FGC25-13310, were exposed to struck-by hazards. The forklift was not taken out of service when it was found to be unsafe and in need of repair. The front headlights were not in working conditions, The accelerator, brake and parking pedals were missing the rubber covers, control levers had no markings on to show its use and purpose.
