29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    (a) Jobsite, 409 Wilkins Road, Rector, PA:  On or about July 24, 2019 an employee who was working on a supported scaffold approximately 12-14 feet above the ground was not protected from falling by guardrails or personal fall arrest system.   The employee fell from the scaffold and sustained the following injuries: fracture to the spine from the cervical through the thoracic, fractured left collarbone and shoulder, fractured rib, tear to the lung and a concussion.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    (a) Facility, 573 McChesney Road, New Alexandria, PA:  On or about July 26, 2019 the employer did not train employees who perform work from scaffolding on the hazards associated with the type of scaffold being used and what can be done to minimize such hazards.
