29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) On or about 07/23/19, on the Brush Mountain Community Activity Center Construction Site, North of Area # 2, 4520 California Road Orchard Park, NY:  Employees used a Ridgid, Model 4040, 8 inch cutoff saw to cut culturel stone. The saw was connected to 120 volt-ac power via a series of extension cords, the first of which was plugged into a temporary power receptacle. The ground prong on the plug of the last extension cord in the series was broken off and stuck in the receptacle of the extension cord.   NO ABATEMENT CERTIFICATION REQUIRED
