Jara Renovations, LLC, conducting work at 7240 Kerfield Drive, Galena, OH 43021: On or about July 24, 2019, the employer did not ensure that employees were using appropriate eye protection such as, but not limited to, safety glasses while conducting construction work with equipment such as, but not limited to, pneumatic nail guns. Employees were exposed to the hazards that could result in eye injuries.

Employees were exposed to approximately 20 feet fall hazard from the roof of the structure to the ground below.

Employees were exposed to approximately 20 feet fall hazard from the roof of the structure to the ground below.
