Jara Renovations, LLC, conducting work at 7240 Kerfield Drive, Galena, OH 43021: On or about July 24, 2019, the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:   A)     J. Jara Renovations, LLC, conducting work at 7240 Kerfield Drive, Galena, OH 43021: On or about July 24, 2019, the employer did not ensure that employees were using appropriate eye protection such as, but not limited to, safety glasses while conducting construction work with equipment such as, but not limited to, pneumatic nail guns. Employees were exposed to the hazards that could result in eye injuries.

Jara Renovations, LLC, conducting work at 7240 Kerfield Drive, Galena, OH 43021: On or about July 24, 2019, the employer did not ensure that employees were protected from fall hazards while conducting roofing activities atop the two story residential structure. Employees were exposed to approximately 20 feet fall hazard from the roof of the structure to the ground below.

Employees were exposed to approximately 20 feet fall hazard from the roof of the structure to the ground below.
