29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   A)     Adan Oswaldo Orellana, conducting work at 6318 Alum Creek Drive, Galena, OH 43021: On or about July 24, 2019, the employer did not ensure that employees were protected from fall hazards while conducting roofing activities atop the one story residential structure. Employees were exposed to approximately 11 feet fall hazard from the roof of the structure to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  A)     Adan Oswaldo Orellana, conducting work at 6318 Alum Creek Drive, Galena, OH 43021: On or about July 24, 2019, the employer did not ensure that employees were adequately trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while conducting roofing activities atop the one story residential structure. Employees were exposed to approximately 11 feet fall hazard from the roof of the structure to the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  A)     Adan Oswaldo Orellana, conducting work at 6318 Alum Creek Drive, Galena, OH 43021: On or about July 24, 2019, the employer did not ensure that the portable ladder, used to access the roof of the one story residential structure, was extended at least three feet above the landing surface. Employees were exposed to approximately 11 feet fall hazard when using the ladder to access the roof in order to conduct construction activities.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  A)     Adan Oswaldo Orellana, conducting work at 6318 Alum Creek Drive, Galena, OH 43021: On or about July 24, 2019, the employer did not ensure that employees, using the portable ladder to access the roof of the one story residential structure, were adequately trained to recognize the hazards associated with the ladders use and to understand the procedures to control or minimize those hazards. Employees were exposed to approximately 11 feet fall hazard due to improper use of the ladder.
