A)     Neil Pence, conducting work at 7352 Yarrow Run Road, Plain City, OH 43064: On or about July 25, 2019, the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to, falls during job site operations.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     A)     Neil Pence, conducting work at 7352 Yarrow Run Road, Plain City, OH 43064: On or about July 25, 2019, the employer did not ensure that employees were adequately trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while conducting roofing activities atop the one story residential structure. Employees were exposed to approximately 10 feet fall hazard from the roof of the structure to the ground below.

Employees were exposed to approximately 12 feet fall hazard from the ladder to the ground below.

A)     Neil Pence, conducting work at 7352 Yarrow Run Road, Plain City, OH 43064: On or about July 25, 2019, the employer did not ensure that employees, using the Werner extension ladder to access pump jack scaffold, were trained to recognize the hazards associated with the ladders use and to understand the procedures to control or minimize those hazards. Employees did not set the ladder at an angle to prevent displacement and were exposed to approximately 12 feet fall hazard from the ladder to the ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    A)     Neil Pence, conducting work at 7352 Yarrow Run Road, Plain City, OH 43064: On or about July 25, 2019, the employer did not ensure that the employee was protected from fall hazards while working on the pump jack scaffold without any fall protection in place. The employee, working in this condition, was exposed to approximately 12 feet fall hazard from the scaffold to the ground below.      Neil Pence was previously cited for a violation of this Occupational Safety and Health Standard (1926.451(g)(1)(vii)), which was contained in OSHA Inspection # 1187154, Citation number 1, Item number 1A, and was affirmed as a final order on December 19, 2016, with respect to a workplace located at1355 Twelve Oaks Court, Blacklick, OH 43004.
