29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) Apartment building located at 1565 Terrell Mill Place, Marietta, GA: On or about 07/25/2019, and at times prior, the employer exposed employees to struck-by hazards while performing roof debris clean-up, in that head protection was not provided to employees located on the ground level directly below employees installing shingles on the roof above.

29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  (a) Apartment building located at 1565 Terrell Mill Place, Marietta, GA: On or about 07/25/2019, and at times prior, the employer exposed employees to amputation hazards from a Jenny Air Compressor, in that the belt guard was missing which exposed employees to moving belts and pulleys on this equipment which were not provided with a guard as employees in close proximity used the walkway to access the roof and clean roof debris.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  (a) Apartment building located at 1565 Terrell Mill Place, Marietta, GA: On or about 07/25/2019, and at times prior, the employer exposed employees to fall hazards of 16-feet, 8-inches from the lower level, in that fall protection was not provided to employees engaged in roofing activities on a 6/12 pitched roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Apartment building located at 1565 Terrell Mill Place, Marietta, GA: On or about 07/25/2019, and at times prior, the employer exposed employees to fall hazards of 16-feet, 8-inches from the lower level, in that the ladder used to access the roof and ground level was not extended at least 3-feet and/or secured to the structure to prevent accidental movement during use.
