29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) Samuel Ortiz Framing located at 132 Kaitlyn Drive, Etna, OH 43062: On and preceding July 29, 2019, employees were loading and securing subfloor to the joists on the second floor a two story residential home. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted by employees.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Samuel Ortiz Framing located at 132 Kaitlyn Drive, Etna, OH 43062: On and preceding July 29, 2019, employees were loading and securing subfloor to the joists on the second floor a two story residential home. The employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards.  The employees were not utilizing proper eye or fall protection which exposed the them to falls up to 18 feet and struck by hazards to the eyes.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Samuel Ortiz Framing located at 132 Kaitlyn Drive, Etna, OH 43062: On and preceding July 29, 2019, employees were loading and securing subfloor to the joists on the second floor a two story residential home. The employer did not ensure that each affected employee used appropriate eye or face protection when utilizing pneumatic nail guns and powered circular saws that can produce flying particles and debris. The following condition exposed the employees to struck by hazards to the eyes.

Framers were installing floor joists and panels at heights that varied from 10-13 feet without the use of fall protection. This condition exposed employees to falls up to 13 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Samuel Ortiz Framing located at 132 Kaitlyn Drive, Etna, OH 43062: On and preceding July 29, 2019, the employer did not ensure that the portable ladder used for access to an upper level floor extended at least 3 feet beyond the upper landing surface. Framers were installing floor joists and panels at heights that varied from 10-13 feet , utilizing an extension ladder for access with approximately 12 inches extending beyond the surface being accessed. This condition exposed employees to falls up to 11 feet.
