29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 07/24/19, on a roofing job site, at a house at 724 Lafayette Ave., Buffalo, NY: An employee engaged in roofing work on the steep roof on the front left side of the house was exposed to a fall of approx. 20 ft. because fall protection was not being utilized. Pitch of the roof was approx. 12 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 07/24/19, on a roofing job site, at a house at 724 Lafayette Ave., Buffalo, NY: Ladder Jack Scaffolding was erected on the front side of the house. Two extension ladders were provided for employees to access the aluminum pik on the scaffold. The side rails of the access extension ladder on the left side was only about 16 inches above the pik, and the side rails of the access extension ladder on the right side was only about 20 inches above the pik. NO ABATEMENT CERTIFICATION REQUIRED
