29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: 1) Front side of the roof: On or about July 24, 2019, the employer exposed employees to fall hazards, in that the 28 foot Werner aluminum extension ladder only extended 2 feet above the upper landing. Employees were using the extension ladder to access and egress to the roof.
