The employer did not provide for frequent and regular inspections of the job site, materials, and equipment. The employer exposed employees to following hazardous conditions: a) Employees were performing residential roofing work approximately 16 feet above the lower level without the use of a fall protective system on a 6:12 pitch roof. b) Employees were using pneumatic nail guns without eye protection.
