29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about July 7, 2019, in the northwest hallway of the building, an employee slipped and fell resulting an in-patient hospitalization type injury that was not reported to OSHA until July 23, 2019, which was beyond the 24 hour reporting requirement.
