29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   a) For employees working at 9757 Tranquility Lake Circle in Riverview, as observed on or about July 30, 2019. Employees were observed working from an aerial lift without the use of a safety harness and lifeline attached to the aerial lift. Employees were exposed to a fall hazard of over 10 feet.    b) For employees working at 9757 Tranquility Lake Circle in Riverview, as observed on or about July 30, 2019. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) For employees working at 9757 Tranquility Lake Circle in Riverview, as observed on or about July 30, 2019. Employees were observed working from an unprotected balcony edge without the use of a fall protection system in use. Employees were exposed to a fall hazard of over 10 feet.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) For employees working at the site as observed on or about July 30, 2019. Employer provided training documents of completed training but certification of training was not provided.  Employees were observed working at heights without the use of personal fall arrest systems in use.
