Exposing them to unprotected fall hazards of approximately 18 feet.

29 CFR 1926.602(c)(1)(vi): 29 CFR 1926.602(c)(1)(vi):Section 603E Industrial truck(s) American National Standards Institute, B56.1-1969, Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed. On the south side of the construction site, the employer allowed the forklift with the forks elevated to be unattended while installing masonry material. The employees were exposed to being crushed by the lift/forks.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii). On the south side of the construction site, employees working from a tubular welded frame scaffold that was not fully planked on all working levels were exposed to unprotected fall hazards. Restivo Masonry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1232658, citation number 1, item number 002a and was affirmed as a final order on 6/22/2017, with respect to a workplace located at 602 Setting Sun Ln., Arlington, Texas 76005.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On the south side of the construction site, the employer did not provide a scaffold ladder where the employee could safely access and egress the tubular welded frame scaffold. The employees were exposed to fall hazards of approximately 18 feet. Restivo Masonry, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 1232658, citation number 1, item number 002b and was affirmed as a final order on 6/22/2017, with respect to a workplace located at 602 Setting Sun Ln., Arlington, Texas 76005.
