On or about July 30, 2019, and at times prior thereto, at a site located at 4708 FM 367 W., Iowa Park, Texas, Rig 129: the employer did not ensure that the employees wore personal protection equipment while engage in oil and gas drilling activities. The employees did not have on Flame-Resistant Clothing (FRCs) exposing them to flash fires.
