29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    On or about 07/29/2019, at 50 Crestwood Court North, Building 1500, Royal Palm Beach, Florida, 33461, employees were exposed to falling 19 feet to ground level while using an extension ladder that was placed in front of a driveway and was not secured or barricaded to prevent from being displaced, to gain access to a roof of an existing building.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or about 07/29/2019, at 50 Crestwood Court North, Building 1500, Royal Palm Beach, Florida 33461, employees making repairs to a roof with a 5:12 pitch and not using any means of conventional fall protection system were exposed to a fall hazard of 19 feet to ground level.    The Garabar, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11) which was contained in OSHA inspection number 1094183, citation number 1 item number 1 and was affirmed as a final order on 01/07/2019, with respect to a workplace located at 2300 Yamato Road, Boca Raton FL 33431.
