29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket.    On or about July 30, 2019, employees were exposed to a fall of approximately 10 feet while performing steel erection activities on a new commercial building located at 1770 Snow, Parma, Ohio 44146. An employee was working from an articulating lift and the employer did not ensure the employee stood firmly on the floor of the basket.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.    Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    On or about July 30, 2019, employees were exposed to a fall of approximately 10 feet while performing steel erection activities on a new commercial building located at 1770 Snow, Parma, Ohio 44146. An employee was working from an articulating lift and the employer did not ensure the employee used a personal fall arrest system.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    On or about July 30, 2019, employees were exposed to a fall of approximately 17 feet while performing steel erection activities on a new commercial building located at 1770 Snow, Parma, Ohio 44146. There were employees working on all areas of the roof and the employer did not ensure that they were using fall protection.
