29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about July 30, 2019, employees of United Structures were exposed to fall hazards while conducting patching work on the top of a commercial building without an adequate form of fall protection.

29 CFR 1926.761(b): Fall hazard training. The employer shall train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. On or about July 30, 2019, United Structures did not certify that the training an employee had received from a previous employer on fall protection would adequately protect them for the fall hazards that they would likely encounter.
