29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems:  On or about 07/23/2019 at the corner of 3rd and Division Street, in Altamont, IL, prior to trenching and excavating efforts to install new sewer lines, an inspection was not conducted by a competent person to prevent employees from exposure to recognizable hazards such as potential cave-in and materials  or equipment falling or rolling into the trench.
