(b) On or about July 30, 2019, on the Baldor PM 2 machine, the employer did not ensure adequate guarding was provided over exposed flanges and nuts, thereby exposing employees to amputation hazards. (c) On or about July 30, 2019, on the Baldor PM 3 machine, the employer did not ensure adequate guarding was provided over exposed flanges and nuts, thereby exposing employees to amputation hazards.

Employers shall base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified. [59 FR 16362, April 6, 1994] (a) On or about July 24, 2019, at the workplace located at 10325 58th Place, Kenosha, WI, the employer did not ensure proper hand protection was selected that prevented worker's hands from becoming entangled on the Baldor PM 1 machine, thereby exposing employees to amputation hazards. (b) On or about July 30, 2019, at the workplace located at 10325 58th Place, Kenosha, WI, the employer did not ensure proper hand protection was selected that prevented worker's hands from becoming entangled on the Baldor PM 2 machine, thereby exposing employees to amputation hazards.
