29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing hazards:   JOB SITE: 247 Brougham Drive, Dardenne Prairie, MO 63368, on or about 7/30/2019, sub-contractor employees were working on the roof of a residential structure.  The general contractor and the subcontractor were on the job site providing material for the project.  The General Contractor did not conduct an inspection of the job site. Employees were exposed to the following:  1.  Employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.  The employee were observed on top of a 6:12 pitch roof, approximately 9' 3"above the ground level without fall protection.  The employees were exposed to a fall hazard. 2.  Employees were observed using a Werner Aluminum extension ladder, for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  The employees were exposed to a fall hazard.
