(a) Frederick Groh, on or about July 31st, 2019, at the location of 28502 Hille Dr., Millbury, OH, 43447: The employer did not have an accident prevention program or plan to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as fall hazards decreasing the probability of employee injury or illness. Employees performing roofing activities were exposed to fall hazards of eight feet, six inches to the ground below.
