29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 30, 2019, and at times prior thereto, the employer did not ensure that the point of operation, ingoing nip points, and rotating parts of the machines that employees operated were guarded. The machines that were found to not have guards included, but is not limited to the following instances: 1) At the standard press brake area, the press brake was operated without guards installed on the sides and in the back to protect other employees in the areas from being exposed to the point-of-operation while the machine is in operation. 2) At the planner mills area, the shaving conveyors were operated without guards installed to protect employees from contacting the moving parts while the machine is in operation. 3) At the planner mills area, the mills were operated without the covers installed to protect the employees from contacting the ingoing nip points while the machine is in operation. Employees were exposed to amputation and caught-in hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: On or about July 30, 2019, and at times prior thereto, at the planner mill area, the employer did not ensure that the sprocket wheel and chain systems, of a planner mill located less than seven (7) feet from the floor level, was enclosed. Employees were exposed to an amputation and caught-in hazards.
