Parking Structures, Inc., 127 East 14th Street, Erie, PA  16503, on or about July 31, 2019 and times prior thereto:  Employees were performing work operations including, but not limited to, masonry pointing activities.  The employee was exposed to a fall hazard of 16.5-feet to the ground below.  Employees were not utilizing fall protection.

Parking Structures, Inc., 127 East 14th Street, Erie, PA  16503, on or about July 31, 2019 and times prior thereto:  Employees were performing work operations including, but not limited to, masonry pointing activities near an unprotected wall opening at a height of 16.5-feet above ground level.  The unprotected wall opening was 36-inch high by 53-inch wide.

The floor opening was 15-inches in diameter and was not covered/guarded.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  A.)  Parking Structures, Inc., 127 East 14th Street, Erie, PA  16503, on or about July 31, 2019 and times prior thereto:  Employees were performing work operations including, but not limited to, masonry pointing activities.  Employees were utilizing chemicals including, but not limited to, Euclid Chemical NS Grout.  The employer's written hazard communication program had deficiencies including, but not limited, the following:  1.)  The program did not designate the person(s) responsible for obtaining/maintaining the SDSs.  2.)  The program did not address how the SDSs would be maintained/accessed at the job site.  3.)  The program did not include procedures to follow if the SDS is not received at the time of the first shipment. 4.)  The program did not include procedure to follow if it is suspected that the SDS is not appropriate. 5.)  The program did not include procedure to follow to determine if the SDS is current. 6.)  The program did not designate the person(s) responsible for conducting hazard communication training.  B.)  Parking Structures, Inc., 127 East 14th Street, Erie, PA  16503, on or about July 31, 2019 and times prior thereto:  Employees were performing work operations including, but not limited to, masonry pointing activities.
