29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without manufacturer's prior written approval. (a) Shamrock Foods Company, at 5005 Kalamath St, Denver, CO 80221: On or around August 1, 2019, the employer did not ensure that an elevated platform attached to a forklift was approved by the powered industrial truck manufacturer. An employee worked from the elevated platform from an unknown manufacturer. This condition exposed employees to Stuck-by and fall hazards up to 20 feet.
