29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: (a) VPM Surgical - The employees working in the Chemical Sharpening and Polishing room were exposed to sulfuric, phosphoric, hydrochloric, and nitric acids when they transferred the scalpels to and from the solution containers and the eye wash bottle was too small and out-of-date, on or about July 31, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a) VPM Surgical - Chemical Sharpening and Polishing Room - The employees were exposed to an electrical shock hazard by the multiple receptacle strip that was attached to the wall behind the Chemical Sharpening hood and used to turn on/off the two (2) sulfuric acid stirrer / hot plates and the polishing ultrasonic unit, on or about July 31, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
