29 CFR 1910.146(c)(8)(ii): When an employer arranged to have employees of another employer (contractor) perform work that involved permit space entry, the host employer did not apprise the contractor of the elements, including the hazards identified:  (a) Public Service Company of Colorado DBA Xcel Energy at 15001 County Road 46, Gilcrest, CO: On 7/25/2019, the host employer did not apprise the contractor of the elements, for work that involved permit space entry. Three contractors were tasked with disassembling a three phase VonRoll transformer. One contractor broke the plane of the transformer with a cordless DeWalt impact wrench in his hand to attempt to disconnect the leads from the bushings. The atmosphere inside of the transformer ignited. The transformer was a permit required confined space. This condition exposed three contractors to a fire and explosion hazard.

The SDS was not provided to subcontractors working on the site. This condition exposed contractors to a chemical hazard.
