29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a- Side of Building, 100 Berlin Crosskeys Road, Berlin NJ: On or about 07/30/19, two company employees were exposed to fall hazards in excess of 12 feet to the ground surface below when working and walking from a supported scaffold system whose walking and working surface was not fully planked. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a- Side of Building, 100 Berlin Crosskeys Road, Berlin NJ: On or about 07/30/19, two company employees were exposed to fall hazards in excess of 12 feet to the ground surface below when working and walking from a supported scaffold system whose walking and working surfaces were not provided with the required guard rail systems and these employees were not provided with a personal fall arrest system as an alternative. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
