29 CFR 1926.251(a)(4): Special custom design lifting accessories for material handling were not marked to indicate the safe working load(s). The Great Lakes Construction Co. on or about August 1, 2019, did not ensure that all custom lifting accessories were marked to indicate the safe working loads and proof-tested prior to use to 125 percent of their rated load. Employees were exposed to crushing hazards while working in and around the area of steel material lifts, when the spreader bar used to pick the steel had not been proof-tested or indicated as safe.

29 CFR 1926.753(d)(2)(ii): Hooks with self-closing safety latches or their equivalent shall be used to prevent components from slipping out of the hook. A) On or about August 1, 2019 the employer did not ensure hooks of a four leg sling equipped with self-closing safety latches or their equivalent were adequately seated on the spreader bar to prevent displacement. Employees utilizing the sling hooks in this condition were exposed crushing and struck by hazards.
