29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2).  Mechanics shop, elevated platform adjacent to mechanics office; on or about 31 July 2019 and times prior thereto; the employer did not provide guardrails across the elevated platform measuring approximately eight-feet above the lower floor. This condition exposed employees to a fall hazard.

29 CFR 1915.159(c)(5): personal fall arrest systems were not inspected prior to each use for mildew, wear, damage, and other deterioration.  Welding shop, on or about 31 July 2019 and times prior thereto; the employer did not ensure that personal fall protection equipment was inspected prior to use. A Titan shock-absorbing lanyard, Model 5122/6FTAF, Inspection number 883740, had visible damage indicating undue stretching. This condition exposed employees to a fall hazard.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit".  Welding shop; on or about 31 July 2019 and times prior thereto; the employer did not ensure that exit signs were posted along the exit route.  The door way leading to the exit route was not marked. This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

29 CFR 1915.55(h): Oxygen, fuel gas pressure regulators and their related gauges were not in proper working order while in use.  Welding shop; on or about 31 July 2019 and times prior thereto; the employer did not ensure that a fuel-gas manifold was in proper working order. The oxygen-propane manifold was missing the face dial glass and had visible damage to the regulator gauge. This condition exposed employees to an explosion hazard.
