Ridges Blvd. Grand Junction, CO 81507: On or before 8/1/19 the creating, correcting and exposing employer did not protect employees from injury in that employees working on the tubular welded scaffold were working on sections that were not fully supported by appropriate mud sills. This condition exposed employees to fall hazards.    (b) ASCO Masonry, Inc at 388 W. Ridges Blvd. Grand Junction, CO 81507: On or before 8/1/19 the creating, correcting and exposing employer did not protect employees from injury in that employees working on the tubular welded scaffold were working on sections that were elevated and supported by wooden scaffold planning and were not secure. This condition exposed employees to fall and struck by hazards.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  (a) ASCO Masonry, Inc at 388 W. Ridges Blvd. Grand Junction, CO 81507: On or before 8/1/19 the creating, correcting and exposing employer did not protect employees from injury in that employees were working on a tubular welded scaffold system that had not been inspected by a competent person prior to allowing employees to use the scaffold system. Hazards were not recognized and corrective actions were not made. This condition exposed employees to fall hazards.

Ridges Blvd. Grand Junction, CO 81507: On or before 8/1/19 the creating, correcting and exposing employer did not protect employees from injury in that employees were working on a tubular welded scaffold system that was missing guard rails at the ends of the sections and along lengths of planking being used as bridges between scaffold sections. These conditions exposed employees to fall hazards ten feet and up to twenty feet.

Ridges Blvd. Grand Junction, CO 81507: On or before 8/1/19 the creating, correcting and exposing employer did not protect employees from injury in that employees working on the roof of a two story multi family development were not wearing fall protection. This condition exposed employees to a fall hazard of 20 feet.
